A long-term physical health condition or LTC (also known as a Chronic Health Condition) is a health problem that requires ongoing management over a period of years or decades.
An LTC is one that cannot currently be cured but can be controlled with the use of medication and/or other therapies.
There are many LTCs and it may help if you understand what some of your people may be dealing with. Some may have one LTC but some are likely to have several, with treatment being with one or more medications which in themselves will have some side effects.
The impact of multi-morbidity is profound. People with several long-term conditions have markedly poorer quality of life, poorer clinical outcomes and longer hospital stays, and are the most costly group of patients that the NHS has to look after.
