From Cornwall to Essex: Leveraging Experience for High-Impact Community Health Delivery

Overview

In partnership with Cornwall Council, iCareiMove was commissioned to deliver a three-year Falls Prevention programme aimed at reducing falls and related injuries in adults aged 65 and over.

Building on our previous success with a digital-only model, this new programme introduced a blended approach that combined in-person classes with digital access via Zoom and pre-recorded sessions — increasing accessibility and reach across the county.

What We Delivered

Our delivery model focused on engaging older adults at risk of falls through a structured and supportive programme. Key elements included:

  • FaME (Falls Management Exercise) classes delivered both in-person and online
  • Functional Fitness MOTs to assess individual needs and monitor progress
  • Weekly newsletters and one-to-one health coaching to maintain motivation and build relationships
  • Opportunities to attend local impact events, fostering community connection

The project was carefully managed using dedicated project and administrative teams, ensuring seamless mobilisation and ongoing delivery. Task tracking via Monday.com, regular briefings, and bi-weekly commissioner updates ensured we remained on schedule and responsive throughout.

Who We Worked With

Collaboration was central to success. We engaged stakeholders across the health and voluntary sectors, including:

  • NHS Specialist Falls Clinics
  • GPs, Physiotherapists, and Occupational Therapists
  • VCFSE organisations such as Volunteer Cornwall and Social Prescribing networks

We also worked closely with Future Fit’s HR and Marketing teams to support training, onboarding, and communications.

What We Achieved

  • On time, on budget: The project is running smoothly and efficiently
  • Positive early impact: Participants are already reporting improved strength, balance, and confidence within weeks of starting
  • Measured improvements: Mid-course assessments show gains in grip strength, functional reach, and sit-to-stand outcomes

Conclusion

This programme not only supports individuals to stay active and independent but also builds long-term capacity within communities. By upskilling local instructors and volunteers, we’re creating a sustainable model that can be replicated elsewhere — including in Essex.

The model’s blend of in-person and digital delivery, backed by strong governance and reporting, makes it particularly well-suited for regions with high demand and diverse accessibility needs.

The Cornwall project draws on a foundation of proven results:

  • Social Return on Investment (SROI): A University of Exeter evaluation calculated a return of £16.47 for every £1 invested in our digital falls prevention model, using the Social Value Engine
  • Quality assurance: Instructor qualifications are verified and monitored through Future Fit’s FLEX LMS, with biannual audits to maintain delivery standards
  • CPD accredited: Our instructor training is endorsed and delivered by Future Fit Instructors and Future Fit Training.
  • Evaluation tools: We use nationally recognised assessments such as WEMWBS, FES-I, and Sit-to-Stand tests to track outcomes

This contract offers a clear blueprint for the upcoming Essex Falls Prevention Service. With similar delivery scope, engagement levels, and reporting requirements, we’re well-positioned to scale our successful Cornwall model to meet the needs of Essex communities.
